OCC Youth Group Activities Consent Form

OTTUMWA COMMUNITY CHURCH YOUTH GROUP
CONSENT FORM 2024/25

In case of a medical emergency, I understand that every effort will be made to contact me. If I cannot be reached, I hereby give Ottumwa Community Church and its youth leaders permission to all healthcare personnel administering medical treatment to use all measures deemed necessary.

I, the undersigned, hereby waive for myself, my heirs, executors, administrators, and assigns any rights or claims for personal injury. I also waive any damages that may occur as a result of this event and any employees, volunteers, or agencies connected with the above organizations as a result of seeking emergency medical attention for my child.

I further understand that I remain ultimately responsible for costs of all medical treatment rendered to my child.

Please provide an email address and/or a cell# where we can contact you with youth group schedule updates and reminders.
During our OCC YOUTH year we take many pictures to be used in the promotion of our OCC YOUTH. Do you permit for Ottumwa Community Church to take your YOUTH's picture and use it? 
During our OCC YOUTH year may your child ride in the youth group vehicles and go on outings? 
In case of emergency, I hereby authorize Ottumwa Community Church and the OCC youth leaders to seek medical attention for my child and release Ottumwa Community Church personnel from all liability. 
I hereby authorize OCC Youth leaders that the following medications may be given to the Participant consistent with medication directions, if the need arises.
