In case of emergency, I hereby authorize Ottumwa Community church and child ministry programs personnel to seek medical attention for my child and release Ottumwa Community Church and child ministry programs and personnel from all liability.
During the year we may take pictures to be used in promotion of our "Community Kids" child ministry programs. Do you give permission for OCC to take pictures of your child?
I understand that it is my responsibility as a parent or legal guardian to obtain and read the appropriate parent handbooks and any supplemental information and provide any additional information needed for each Community Kids Progam or special event my child is enrolled in. Also, I agree to update and resubmit this form for any changes in the information provided as soon as possible.
