During our AWANA year we take many pictures to be used in the promotion of our AWANA CLUB and for our end of the year slide show. Do you give permission for Ottumwa Community Church to take your child's picture and use it?

Emergency AUTHORIZATION (Required):

In case of emergency, I hereby authorize the AWANA program at Ottumwa Community Church to seek medical attention for the child listed and release the AWANA program and Ottumwa Community Church from all liability.
